Appointment policy agreement form
Loc’d by Her welcomes you with open arms and tons of care! We strive to deliver exceptional service and appreciate your business. To maintain the highest standards and ensure fairness for all clients, we have established the following policies. Please be advised that these that these policies are subject to change at any time and are governed by law. Failure to abide by these policies will result in the cancellation of your service with Loc’d by Her, you are agreeing to these terms. We reserve the right to refuse service tour discretion. Thank you for your understanding and cooperation and as always we appreciate your support!!
new clients
All new clients are required to book a consultation. This consultation fee goes does not go towards your first service. Please not that, even if you decide not to proceed with any services, the consultation fee paid is non-refundable. If after your consultation you choose not to proceed with service and later change your mind, a new consultation fee will be required to book future services. Your first serve will be booked at your consultation, please be prepared to pay in full for your future appointment. In the event that the client chooses to book their first service manually online rather than waiting until their consultation, please note that if the correct service or add ons were not chosen, it may result in a reschedule or cancellation. The new client consultation must be booked at least one day prior to the first service. New client consultations and the first service may NOT be booked on the same day. If a client shall book a full service appointment without booking a consultation first, the appointment will be canceled and the consultation fee will be forfeited.
PLEASE NOTE!! If a client has been previously serviced by Loc’d' by Her, but has not been serviced in 6 months, they are considered a new client and must book a new client consultation.
reschedules
Clients may reschedule their appointments up to 48 hours prior to their scheduled service time. Any rescheduling request made within 48 hours will result in forfeiture of the entire payment.
late arrival
All appointments have a 15-minute grace period. If a client arrives more than 15 minutes late, the appointment will accrue a fee. Anything after 30 minutes will be cancelled and charged 100% of the service fee. All fees must be paid prior to rebooking.
no call, no show
Clients must notify Loc’d by Her at least 2 hours prior to their scheduled appointment if they are unable to attend. Notification must be made via call. Failure to provide this will be considered a No Call, No Show. Clients who do not provide the required 2-hour notification will forfeit the entire payment of their scheduled service. This charge is necessary to compensate for the time and resources allocated to the appointment. Any disputes regarding the No Call, No Show charge will be addressed through legal action. We are committed to fair and transparent dealings and will take necessary steps to resolve dispute in accordance with legal guidelines.
extra guests
Clients are allowed one extra guest, including children, per appointment. Children must be supervised during your visit.
payments
All appointments have a deposit that must be paid when booking an appointment. The remaining balance can be settled using cash, card, or Zelle. I do not accept Cash App or Apple Pay via text message. I do accept Tap to Pay. All services require 1/3rd of the balance to be paid upfront.
refunds
Refunds are only available if the following conditions are met:
The refund request has to be made within 48 hours of booking the appointment and the scheduled appointment is more than 30 days away from the time of booking. If your appointment is scheduled within 30 days of the booking date, refunds will not be granted under any circumstances.
Non Refundable payments are payments made to secure your appointment if the request for a refund does not meet the above criteria.
overdue retwist
To ensure the best care and maintenance of your locs, it is important to adhere to a regular retwist schedule. 4-6 weeks is the best recommendation for mature locs. If a client is overdue for a retwist service, an additional fee will be applied based on the duration of the overdue period. See Below:
3-4 months overdue: Additional $25
5-6 months overdue: Additional $50
7-8 months overdue: Additional $75
9 months or more overdue: Additional $100
Anything over a year must do a consultation to access the condition of the locs and determine the appropriate service and fee
add-ons
Please ensure that all necessary add-ons are correctly booked when booking your appointment. If an add-on is not selected, it will not be performed during your appointment time. For example, if you would like to receive a style (two-strand twist, petals, etc) but did not select the complex style when you booked your appointment, it will not be performed. Failure to add loc count add ons will be subject to rescheduling or cancellation.
before/after hours fee
All appointments booked before the shop opens, after the shop closes, or on a Sunday or Monday will have a fee attached to them. When booking your appointment, if you need a special appointment, please make sure that you add the appropriate fee for any appointments that are on a Sunday or Monday and that are before or after hours.
same day/squeeze ins
Same day or squeeze in appointments (appointments made in less than 24 hours) will have a few attached to the appointment.
photo/ video
During your appointment, there will be recording of the process of doing your hair. If at any point, you are uncomfortable or would not like me to record or take pictures, please do not hesitate to let me know.
